Schedule Your Appointment

Welcome to Advanced DermCare’s appointment request area of our website.  The next few pages will give you the opportunity to request an appointment by providing us with a date and time that suits your needs.  This area should be used to request appointments that are not for urgent problems that need to be seen immediately.  If you feel that your problem must be addressed immediately, please call our office at 203.797.8990 instead of using this form.  

Once you have submitted the necessary information for your appointment request, our staff will search for some times that best meet your needs.  Then, within one business day, someone from our office will contact you with several possible appointments that fit the days and times that you have requested. When you decide which time slot works for you, we will schedule the appointment. Sending us your requested times using this website does not create an appointment for you – the appointment won’t be scheduled until we speak with you on the phone.

As with any electronic system, occasionally we may experience problems receiving appointment requests.   So, if you have sent a request for an appointment and have not heard back from us within 48 hours, or if your problem becomes more urgent, please call us at 203.797.8990.  By clicking apply below, you indicate that you have read, understood, and agreed to comply with these terms.   Thank you.  We look forward to seeing you in the office!
